Membership Application
Fill out the form below to apply for membership. The one-time membership fee is $100, payable at the store. You can also pick up a paper form in person.
Two payment options:
• Pay $100 in full. Benefits begin immediately.
• Installment payments of $25/month. Member benefits begin when the full $100 is paid.
Payment is accepted at the store.
